HIPxChange: Tools for Health System Change and Population Health Improvement

HIPxChange is a web portal that provides evidence-based tools for health system change, sponsored by the Health Innovation Program at the University of Wisconsin School of Medicine and Public Health. The goal of HIPxChange is to accelerate the translation of knowledge into clinical practice to improve healthcare delivery and health outcomes.

The toolkits available on HIPxChange are free to the public, and can be used by clinicians, health care administrators, program managers, and researchers, and span a variety of topic areas, such as:

  • Increasing patient & stakeholder engagement
  • Reducing hospital readmissions
  • Improving care for chronic conditions, and more

Many of the toolkits provide implementation information for programs or interventions that the authors have published in peer-reviewed journals, helping bridge the gap from research to practice.
