Invitation to Participate: Clinical Sustainability Assessment

You are invited by the Center for Public Health Systems Science at Washington University in St. Louis to participate in a Clinical Sustainability Assessment. The purpose of this assessment tool is to collect information about your clinical practice or intervention regarding key elements that contribute to sustainability. The results of this survey will provide you with important information about the factors that are likely to help sustain your practice efforts over time. The results can also be used to inform future sustainability planning efforts.

To access the Clinical Sustainability Assessment Tool, simply click on the link below or copy and paste the URL into your internet browser. The tool should take approximately 15 minutes to complete. You will have the ability to save your work and can use the link provided to return to the assessment later.

Your participation is voluntary. The results of this survey may be included in reports, published scientific research journals, and/or presentations at conferences, but none of these will disclose your identity. In return for your time, we will provide you with an emailed report, summarizing your results. Those individuals who complete the assessment can also choose to enter into a drawing to win one of five $50 gift cards as a thank you for your time, feedback, and assistance.
